LASIK or Lens Exchange After 45? How to Think About It
After 45 the eye's natural lens starts to fail at near focus. Why that changes the LASIK conversation, and when refractive lens exchange is the better discussion.

In your twenties the question is simple: you are nearsighted, the cornea can be reshaped, LASIK or SMILE fixes the focus. In your late forties a second thing is happening that laser surgery cannot touch.
What changes in the mid-forties
The natural lens inside the eye flexes to focus up close. With age it stiffens, and near focus fades: this is presbyopia, and it is why people who never needed glasses start holding menus at arm's length. The same lens will, years later, turn cloudy and become a cataract.
What LASIK can and cannot do at this age
LASIK still corrects distance vision well in a healthy eye at any adult age. It does nothing for the stiffening lens, so reading glasses remain, and if your distance correction is removed you may notice near blur more than before. It also leaves in place a lens that will eventually need cataract surgery.
What lens exchange does instead
Refractive lens exchange replaces the ageing lens with an artificial intraocular lens. Depending on the lens chosen it can address distance and near together, it treats prescriptions too high for the laser, and that eye will not develop a cataract. It is also surgery inside the eye, so its risks are different from LASIK's and include, rarely, infection and retinal detachment.
A rough guide
Under about 45 with a stable prescription and healthy corneas: laser surgery is usually the discussion. Over about 50, or with early lens changes, or mainly bothered by reading glasses: lens exchange usually is. Between the two it is a judgement made from your measurements, your prescription and what you want to stop wearing.
